New in 2021: All clients should submit a completed AG-257 (one-time registration) for a client number to access plant diagnostic services.

For a specific test that does not have a specified form, please use FORM D1178 – General Diagnostic Service Form and note the requested test in the SPECIFIC TEST REQUEST (bottom right corner of the form) area.  Specific requested testing may incur additional charges.

Samples can be submitted via courier, mail (preferably no more than 3-day delivery), or by walk-in.

Walk-in hours: 9am – 4pm, closed 12pm-1pm
