Submission Forms and Instructions

New in 2024: A NEW Plant Disease Diagnosis Form (D-1178) and Plant Nematode Detection Form (D-827) will take effect June 1, 2024. Please download and use the new versions of the forms from the link above.

New in 2021: All clients should submit a completed AG-257 (one-time registration) for a client number to access plant diagnostic services. The department contact is ‘COURTNEY OVERBY’ and the department contact email is ‘courtney.overby@ag.tamu.edu’.

All downloadable forms are in PDF format. You may need to get Adobe Acrobat Reader (free reader) to be able to access these forms. These are fillable PDF forms.

For a specific test that does not have a specified form, please use FORM D1178 – General Diagnostic Service Form and note the requested test in the SPECIFIC TEST REQUEST (bottom right corner of the form) area.  Specific requested testing may incur additional charges.

Samples can be submitted via courier, mail (preferably no more than 3-day delivery), or by walk-in.
