Submission Forms and Instructions

New in 2024: A NEW Plant Disease Diagnosis Form (D-1178) and Plant Nematode Detection Form (D-827) will take effect September 1, 2024. Please download and use the new versions of the forms from the link above.

New in 2021: All clients should submit a completed AG-257 (one-time registration) for a client number to access plant diagnostic services. The department contact is ‘COURTNEY OVERBY’ and the department contact email is ‘courtney.overby@ag.tamu.edu’. For any other inquiries, please email plantclinic@tamu.edu.

All downloadable forms are in PDF format. You may need to get Adobe Acrobat Reader (free reader) to be able to access these forms. These are fillable PDF forms.

For a specific test that does not have a specified form, please use FORM D1178 – General Diagnostic Service Form and note the requested test in the SPECIFIC TEST REQUEST (bottom right corner of the form) area.  Specific requested testing may incur additional charges.
